my dr. said to walk with a back pack, starting with just a pound or two and working up to 10#, so I put soup, veggies cans, but if I go over 3# it makes my feet hurt, so I just use the lower weight. I got one that sits fairly high on my back so it won't drag my shoulder back and down.
My son got one a while back and sometimes even works wearing it. I am seriously considering buying one for myself. His can be weighted from 1-20 pounds. Vest distributes the weight more evenly than a backpack would..or so it seems to me. I've lost 106 pounds, so I'm not carrying as much weight as I was when I was losing it, and am struggling with a plateau. Was thinking the added calories I'd use with weighted vest might do the trick. Bad ankles, knees, etc might not make this a good option for everyone. I agree that you might want to talk to your doctor about this first to get an opinion specific to your needs.
